RN/LVN Provide medical care and support to patients who are recovering from illness, surgery, or managing chronic health conditions, all within the comfort of their own homes.
Support and assist individuals who need help with daily activities, often in their own homes. HHAs play an essential role in ensuring that patients can live as comfortably and independently as possible.
Delivers rehabilitation services in a patient's home environment to help them regain strength, mobility, and independence following an injury, surgery, or managing chronic conditions. Work directly with patients to improve their functional abilities, reduce pain, and prevent further physical limitations.
Focuses on helping individuals regain or develop the skills they need to perform activities of daily living (ADLs). Helps patients that have difficulty performing everyday tasks independently.
Focuses on treating patients who have communication, speech, language, swallowing, or cognitive issues due to various conditions such as stroke, brain injury, developmental disorders, or aging-related conditions like dementia. Speech therapists (also known as speech-language pathologists, or SLPs) provide personalized treatment plans to help patients regain their ability to communicate, eat, and drink safely, improving their overall quality of life at home.
A crucial role in supporting patients and their families during recovery, rehabilitation, or while managing chronic illnesses. They focus on addressing the emotional, social, and financial needs of patients while coordinating care and advocating for services. In the home health setting, social workers help individuals navigate the challenges of receiving care at home, ensuring they have access to the resources, support systems, and care services they need to maintain their well-being
Provide specialized nutrition care and counseling to patients in the comfort of their homes, helping individuals manage health conditions, improve their overall well-being, and prevent or treat nutritional deficiencies. They assess, plan, and monitor the nutritional needs of patients, especially those with chronic conditions, disabilities, or recovering from surgery or illness.
Designed to assist patients & their families in finding appropriate housing and care options based on the individual’s health, needs, and preferences. Provide guidance, support, and information about pediatric & senior living options, such as independent living, assisted living, memory care, nursing homes, and other types of residential care facilities. We serve as advocates, helping families navigate the complex and often overwhelming process of finding the best care solution for aging loved ones.
