PLACEMENT

Our Goal

WE serve as advocates, helping families navigate the complex and often overwhelming process of finding the best care solution for loved ones. Whether it's a child or an aging adult we understand the guidance and support that is needed to overcome the difficult decisions and challenges. Isla Home Health will set up tours to the facilities with you to ensure that you make an informed decision. We build rapport with facilities so ensure that we offer only reputable places for your loved one to call home.

  • Personalized Assessments: Conduct an in-depth evaluation of the client's health, personal preferences, lifestyle, cognitive function, and level of independence. This helps determine which type of care and facility would be most appropriate.
  • Medical and Social Considerations: The agency considers the medical conditions, including chronic illnesses, mobility challenges, cognitive impairment (such as dementia or Alzheimer’s disease), and social needs (e.g., desire for social interaction or specialized programs).
  • Financial Assessment: Placement agencies often assess the individual’s financial resources to ensure that the chosen facility or care option is affordable and aligns with the person’s budget, factoring in insurance and payment options.

TYPES OF HOUSING OPTIONS

  • Independent Living: For seniors who are largely independent but may need minimal assistance with daily tasks, independent living communities offer a social environment with amenities like housekeeping, meals, and transportation.
  • Assisted Living: For those who need help with activities of daily living (ADLs), such as dressing, bathing, or medication management, assisted living communities provide the necessary support in a more independent living environment.
  • Memory Care: For seniors with Alzheimer's disease, dementia, or other forms of cognitive impairment, memory care facilities offer specialized care and structured environments to ensure safety and address unique needs.
  • Nursing Homes/Skilled Nursing Facilities: For those who need round-the-clock medical care or rehabilitation services, nursing homes provide skilled nursing care, physical therapy, and assistance with daily living.
  • Respite Care: Placement agencies may also help families find temporary care options, such as respite care, to give caregivers a break while their loved one is taken care of in a short-term setting.
